Incident Investigation Using Root Cause Analysis
In the unfortunate event of an incident, the Incident Investigation is a vital part of ensuring that an event does not reoccur. At Trinity Safety Group, we use proven, structured analysis techniques that apply to one-time or repeated incidents. Using Root Cause Analysis, we investigate an incident beyond the immediate factors (causal factors) and identify the root causes (the exact behaviors, environmental factors and decisions) that lead up to the incident. Using these root causes of the incident, we are able to develop a comprehensive report that includes remedial actions to implement and help ensure that the incident will not reoccur.
The tools that make up our Root Cause Analysis include: fault-tree analysis, events and causal factor analysis, Management Oversight Risk Tree (MORT) and Human Performance Analysis. We will use these tools to gather, identify and evaluate the root causes to provide remedial actions of the incident.
Root Cause Analysis Process involves the following steps:
- Gather and Organize the Incident Data
- Identify Causal Factors from the Incident Data
- Identify the Root Causes of the Incident
- Develop Effective Remedial Actions
- Complete a Report of Data Collected, Causal Factors, Root Causes and Remedial Actions
- Assist in the Implementation of Remedial Actions (i.e. Training, Site Assessments, etc.)
